I respect your spirit for good health at this age.
If you have faced sudden rise in your wait, then you need good physician. get your medicines checked as it can be side effect of the same.

beyond that if you have diabetes and blood pressure then:

have papaya as your breakfast instead of tea..if you are habituated for tea...then go for sugar free, and have 1 or 2 small khakhra in break fast.
do walk on regular basis for at-least 30 min.
try to have more green vegetables it has no added sugar and gives you high energy with low fat...

Avoid oily and deeply fried food...and have only homemade healthy food. you can also try yoga and pranayam if healthy otherwise.
